Job Summary 

The Advanced Practice Provider (Nurse Practitioner/Physician Assistant) plays a crucial role within the ambulatory care environment, collaborating closely with a multidisciplinary healthcare team. The Advanced Practice Provider is responsible for delivering comprehensive care within an Internal Medicine practice, focusing on adult patients.

Essential Functions

  • Conduct assessments, diagnoses, and management of patients in a primary care setting.
  • Provide preventive health services, including routine visits, screenings, and immunizations, and address acute and complex care needs.
  • Collaborate with an interdisciplinary healthcare team to ensure optimal patient outcomes.
  • Evaluate patient risk factors, encompassing physical, socioeconomic, and psychosocial aspects.
  • Gather and document comprehensive patient histories and perform thorough physical examinations.
  • Order, conduct, and interpret necessary diagnostic tests, responding to results that require further evaluation.
  • Develop differential diagnoses and engage with collaborative physicians as needed.
  • Coordinate care with specialists when necessary.
  • Formulate and document individualized plans of care based on patient needs.
  • Utilize evidence-based guidelines to create dynamic care plans applicable across various patient scenarios.
  • Implement and adjust care plans based on patient responses and diagnostic findings.
  • Prescribe appropriate diagnostic and therapeutic interventions.
  • Perform in-office procedures within the scope of practice.

Documentation:

  • All outpatient visits must be documented in the EPIC system using the SOAP format.
  • Ensure timely documentation that meets compliance standards, capturing essential patient information including history, presenting issues, physical exams, diagnoses, and treatment plans

Additional Responsibilities:

  • Review diagnostic study results promptly and communicate follow-up plans to patients. Address messages and inquiries through the Epic portal.
  • Review specialist consults and inpatient summaries to maintain comprehensive patient records. Provide coverage for in-basket messages as needed.
  • Provide on call coverage as assigned by Medical Director Communication and Collaboration:
  • Maintain ongoing communication with office staff and the interdisciplinary healthcare team, ensuring effective collaboration with patients and their care teams.

Quality and Productivity:

10%

Engage with the population health team to enhance patient access and outcomes, striving to meet annual quality metrics as defined by the organization.

Professional Development:

Demonstrate professional practice behaviors, including mentoring and educating students. Seek opportunities for involvement in research and evidence-based practice analysis. Maintain national board certification, state licensure, and required continuing education.

Required Qualifications 

  • Minimum of Master’s Degree in Science or related field.
  • Minimum of 2 years working experience as an Advanced Care Provider in a clinical care environment.

Additional Information 

This position is classified as exempt grade N. Compensation for this grade ranges from  $  90,430.00 to  $135,640.00 per year . Please note that the offered rate for this position typically aligns with the minimum to midrange of this grade, but it can vary based on the successful candidate’s qualifications and experience, department budget, and an internal equity review.
