Job Summary

Provides medical-based case management services including assessment, development of intervention plans, counseling, resource referral and linkage, service monitoring and tracking for HIV infected individuals receiving clinical care at the Partnership Comprehensive Care Practice.  Responsible for medical case management of assigned adult patients seen in the Partnership Clinic.

Essential Functions

  • Provide social work case management services designed to assist patients and their families deal with psychosocial, emotional, financial, and environmental sequel of HIV disease and disease progression
  • Respond to physician referrals and client requests for case management assessment and intervention within required response time
  • Provide patient and family education on psychosocial and adaptive aspects of the disease process and offer education on accessing relevant services and resources
  • Perform initial comprehensive case management assessment and develop a plan of intervention that addresses identified need for crisis intervention, solutions on grief and bereavement counseling, continuity of care planning and coordination of needed community services
  • Provide patient and family education on psychosocial and adaptive aspects of disease processes and offer education on accessing relevant services and resources
  • Provide advocacy services to ensure linkage to appropriate services or resources for patients who are eligible for such
  • Participate in interdisciplinary care planning and collaboration to ensure effective and efficient coordination of ambulatory care resources
  • Perform all administrative tasks related to caseload such a medical record documentation, entitlement applications, department statistics, continuity of care referral paperwork, team reports, etc
  • Participates in Division and grant related clinical and administrative meetings and activities

Required Qualifications

  • Master’s Degree in Social Work or related field from an accredited school
  • 2 plus years of experience or internship in a health care setting or AIDS service organization
  • Must obtain a certification in HIV Case Management during first year of employment and maintain certification requirements
  • Demonstrated knowledge of HIV disease, disease transmission and progression
  • Excellent communication and interpersonal skills along with customer service orientation and problem-solving ability required
