Job Summary

Plan and deliver the nutritional care of HIV patients in accordance with providers’ diagnosis and medical nutritional therapy guidelines. Keep interdisciplinary team members up to date with current nutrition standards of care related to primary care of adults with HIV disease.

Essential Functions

  • Counsel and educate patients in current nutritional guidelines with practical real life applications using effective counseling skills, innovative group classes, community resources, and educational tools.
  • Provide nutritional care through comprehensive nutrition assessments and Medical Nutrition Therapy (MNT), diagnose nutrition-related problems, determine and implement appropriate targeted interventions, and monitor and evaluate outcomes.
  • Establish and maintain effective working relationships with all disciplines to provide a unified team approach.
  • Develop and implement a plan of care, with patient input, based on an assessment of nutritional status and current evidence based guidelines.
  • Maintain accurate records of all patient interactions and recommendations in the patient’s medical record as well as in the CareWare database.
  • Assist in the development, design, and improvement of patient education materials.
  • Participate in continuous quality improvement activities to ensure optimal nutrition care outcomes are achieved.
  • Assumes active role in ensuring patient satisfaction with nutritional service. Makes recommendations and actively seeks resolutions to patient’s concerns to ensure patient satisfaction.
  • Participate in multidisciplinary team meetings, such as Complex Case Conference and CQI Committee, to improve patient outcomes. • Complete referrals for home bound meals when medically indicated.
  • Complete documentation for nutritional supplement request where indicated.
  • Submit accurate and timely billing charges.
  • Communicate with physicians and other health care personnel by participation in multidisciplinary team huddle during each daily clinical session as well as via documentation and EHR task.
  • Serve as an educational resource in nutrition and provide in-service educational programs to medical, nursing and other health care staff as required.
  • Maintain nutrition knowledge base through participating in continuing education programs. - (Essential)

Required Qualifications

  • Minimum of Bachelor's Degree n Food and Nutrition or the equivalent combination of education and work experience. 
  • Minimum of 3 years of experience. 
  • Basic education as set forth by the Academy of Nutrition and Dietetics. B.S. degree in Food and Nutrition related field plus successful completion of dietetic traineeship, internship, coordinated undergraduate program or an advanced degree in the field of Dietetics with qualifying experience; must be a Registered Dietitian (RD) by the Commission on Dietetic Registration.

This position is classified as a Full time Exempt with a grade of L [$62,630.00 - $  93,950.00]. The offered pay is dependent upon internal equity and the successful candidate’s competencies, education, and experience.
