Job Summary

The Medical Case Management Services of the Partnership Comprehensive Care Practice (PCCP) Outpatient Clinic serves an essential role as part of a multi-disciplinary team that provides patient care and education for adult patients living with HIV. Under the direction of the Medical Case Management Program Manager, this position will provide a range of trauma informed and patient-centered activities focused on improving health care outcomes. Duties include, but are not limited to: Initial and ongoing assessment of service needs;, timely and coordinated access to medically appropriate levels of healthcare;, collaboration with care providers and medical support staff to support antiretroviral therapy adherence; accompanying a client to a medical appointment with client’s consent, client-specific advocacy and/or review of utilization of service;, and positively promotes the Mission, Vision and Values of the Partnership Comprehensive Care Practice and Drexel Medicine.

Essential Functions

  • Support a caseload of approximately 55 patients to achieve individualized wellness goals through the completion of comprehensive assessments and care plans. This includes the provision of client-centered activities focused on improving health outcomes in support of the HIV care continuum. 
  • Evaluate and update the assessment and care plan at least twice a year to address changes in health needs, assess appropriate level of care and modify goals based on patient preference and readiness for change. 
  • Collaborate with providers, other disciplines of the care team, and specialty practices to ensure all team members are aware of patient’s needs and health outcomes. 
  • Engage in dialogue to address any barriers that negatively impact health outcomes. This may include behavioral health, benefits, transportation, substance use, childcare, safe/affordable housing, medication adherence, and/or any specific challenges faced by marginalized populations.
  • Coordination and linkage to needed community services including monitoring efficacy and outcomes. 
  • Coordination with the Outreach team to engage lost to care patients and/or to prevent lost to care patients.
  • Maintain the weekly productivity requirements.
  • Document all services in the electronic medical record in a timely manner.
  • Enter all productivity units into the CAREWare Database. 
  • Work among a team of eight case managers to share the resource database and provide cross coverage as needed.
  • Participate in the supervision sessions and team meetings as required. • Other tasks as deemed necessary. 

Required Qualifications

  • Minimum of Master's Degree in Council of Social Work Education (CSWE) or the equivalent combination of education and work experience. 
  • Minimum of 1 year preferably in HIV/AIDS organization.
  • Must obtain certification in HIV Medical Case Management through Philadelphia’s Division of HIV Health during the first year of employment and maintain certification requirements.
  • Experience with mental health and addiction services preferred.
  • Fluency in both Spanish and English languages required.
  • Latino intercultural competency.
  • Excellent organizational and problem-solving skills, with the ability to manage competing priorities.
  • Attention to detail.
  • Knowledge of and experience supporting patients of diverse identities and backgrounds, including race, gender, and LGBTQIA+ as well as deep interest in advocating for marginalized communities and improving diversity, equity and inclusion required.
  • Commitment to learning and delivery of trauma-informed care is essential.
  • Must be able to stand at least 25% of the day and walk within and between buildings.
  • Familiarity with electronic medical record systems, and proficiency with standard computer systems, including Microsoft Office and Zoom are required.

Additional Information
This position is classified as Exempt, grade K. Compensation for this grade ranges from $ 54,630 to $$ 81,940 per year.  Please note that the offered rate for this position typically aligns with the minimum to midrange of this grade, but it can vary based on the successful candidate’s qualifications and experience, department budget, and an internal equity review.
