Job Summary

The Community Health Worker (CHW) is an integral component of the patient care team for the family planning program in the Section of Adolescent Medicine at St. Christopher's Hospital for Children. CHW’s serve as liaisons between health educators, communities, social services and other healthcare professionals. CHWs advocate for the reproductive healthcare needs of individuals ages 13 through 21, educate on the availability of healthcare services and conduct outreach and linkage to resources for teens and young adults. CHW’s help patients with complex health conditions navigate healthcare systems.

Essential Functions

  • Adherence to HIPAA.
  • Able to deal with patients who experience adversities and handle confidential or discreet diagnosis.
  • Represent a positive and professional image of the practice to patients, families, community institutions and external customers.
  • Responsible for establishing trusting relationships with patients and their families while providing general support and engagement.
  • Providing ongoing follow-up, basic motivational interviewing and goal setting with patients/families.
  • Skilled in identifying problems and recommending solutions.
  • Helping families to coordinate appointments as well as encourage patients/families to attend appointments.
  • Knowledgeable about community resources appropriate to needs of patients/families
  • Sensitivity to unique/non-traditional life circumstances and the ability to support members to improved healthcare outcomes.
  • Updates and documents in electronic patient record and electronic patient management system, in an accurate, timely and efficient manner.

Required Qualifications

  • High School Diploma or GED
  • 1-2 years of healthcare experience

Preferred Qualifications

  • Some college credits preferred
  • Certification as a Community Health Worker preferred
