Job Summary

The Care Coordinator is responsible for the completion of clinical and administrative tasks in primary care to ensure that patients receive health care that is competent, caring, and meets with a high degree of satisfaction.  The Care Coordinator is a key linkage person in our Care Management Program, who helps patients navigate between services and resources at the site.

Essential Functions

  • Team members are expected to uphold the health center mission by assuring that patients receive health care that is competent, caring, and meets with a high degree of satisfaction. In addition, staff must support FPCN's commitment to the creation of a trauma-informed system of care that continually recognizes and responds to the impact of traumatic stress on all those who have contact with the organization, including children, adults, families, caregivers, and staff.
  • Other duties as assigned

Required Qualifications

  • High School Diploma or GED
  • 3 years of relevant experience working as a Medical Assistant
  • Current Medical Assistant Certification in Commonwealth of PA

Preferred Qualifications

  • Experience working within a community-based health center preferred
  • Exemplary organizational skills and ability to prioritize
  • Exemplary customer service skills
  • Highly responsible and reliable as well as courteous

This position is classified as Non-Exempt, grade H. Compensation for this grade ranges from $17.14 - $25.71 per hour. Please note that the offered rate for this position typically aligns with the minimum to midrange of this grade, but it can vary based on the successful candidate’s qualifications and experience, department budget, and an internal equity review.
