Senior Community Health Worker Peer, Community Outreach Team

Job Summary
We are seeking a compassionate and experienced individual to fill the role of Senior Community Health Worker Practitioner (CHWP) at both the Community Outreach Team (COT) and Youth Advisory Board (YAB) programs, affiliated with the Center for Nonviolence and Social Justice at Drexel University's Dornsife School of Public Health. The successful candidate will split their time between providing support to the Youth Advisory Board, partnering with community organizations, and remote work, ensuring the delivery of comprehensive community health services.
This position is grant-funded; employment is contingent upon the continued availability of those funds.
Essential Functions
- Outreach and Engagement: Conduct in-person outreach and engagement activities at various partner sites, including YouthBuild, Students Run Philly Style, and other youth-focused organizations. Develop trusting relationships with potential clients based on lived experience.
- Assist in identifying and participating in at least 1 resource fair and other tabling event per month.
- Program Development: Assist in the development and implementation of interventions at partner sites, including the CYPHER (Community of Young People Healing, Experiencing, and Rebuilding) groups, which are culturally relevant trauma recovery groups.
- Supervision and Mentorship: Provide guidance and mentorship to COT's CHWPs and to CNSJ interns.
- Act as a key contact for staff at partner sites and contribute to the development of site-specific interventions.
- Documentation and Reporting: Maintain accurate records of patient interactions, program activities, and outcomes.
- Ensure compliance with funding requirements and organizational guidelines.
- Training and Onboarding: Facilitate training activities for new CHWPs and interns, including shadowing, orientation, and evaluation of internship experiences.
- Youth Advisory Board Coordination: Facilitate recruitment, onboarding, and retention of YAB members; planning and facilitating bi-weekly meetings, workshops, and events; supporting youth leadership and skill development; providing mentorship and ongoing support to YAB members; and serving as a liaison between youth members and organizational leadership to ensure youth voices are incorporated into programs, policies, and decision-making.
Required Qualifications
- Minimum of a HS Diploma or GED
- Minimum of 4 years of experience.
- Pennsylvania Peer Specialist and Community Health Worker Peer (CHWP) Certification required
- Minimum of 4 years’ experience providing peer services to violently injured people

Additional Information
This position is classified as Exempt, grade I. Compensation for this grade ranges from $45,330 to $68,000 per year. Please note that the offered rate for this position will vary based on the successful candidate’s qualifications and experience, department budget, and an internal equity review.
